Hello Meads Mill Parents and Students! This is a reminder that over the summer it's time to begin logging in and electronically signing your sports forms in Final Forms for the 2024-25 school year. Whether you are an incoming 6th grader, new to Final Forms, or a returning athlete, click on this webpage link to set up new accounts, review and update your information, and see payment information. Payments are due prior to the first contest of the season.
Per MHSAA rules, a "current" sports physical must be dated after 4/15/24 for the upcoming school year in order for students to practice or participate. Sports physicals must be uploaded to Final Forms and be approved before a student can try out or practice for any sport.
The Meads Mill office will be closed beginning June 27 and will reopen August 5. If you should have questions or concerns, feel free to call the office beginning August 6 at 248-344-3700.
